1. Don't be embarrassed, nervous or shy to call.

2. Call the office at 1-860-263-7916 between the hours of 9:00am-5:00pm (Bilingual staff)

3. Give the receptionist your name and notify him / her that you would like to schedule an appointment for intake.

4. Provide the receptionist with the date your available and she will schedule your time.

5. Plan to arrive 15 minutes prior to your scheduled time.

6. If for any reason you cannot make your appointed time, please call to cancel.

7. Upon arrival, please bring your identification as you will be checked in by security, mask required.

8. If you do not have any form of identification, please let the receptionist know when booking your appointment.

9. Be prepared to be with us for a minimum of 1 hour. 

10. Bring all necessary documentation and anything that you may have that will better help us assist you.
